Eufaula Middle School Enrollment Checklist

 

Thank you for choosing Eufaula Middle School.  All new students and those who have been legally withdrawn from the district must complete the enrollment process.  Students must live with a parent or legal guardian who is a resident of the Eufaula Public School District or obtain an approved out-of-district transfer.

Items to Bring with you to Enrollment:

  • Birth Certificate
  • Shot Record
  • Proof of Residency
  • Social Security number
  • Withdrawal Grades
  • CDIB Card
  • Medicaid Card

 

Forms you will be asked to complete:

  • Enrollment Card - phone number, address, family doctor, place of employment, last school attended
  • Course Request Form
  • Lunch Form
  • Title VII Student Eligibility Certification for Indian Education/ Johnson-O’Malley Enrollment Sheet
  • Home Language Survey
  • Transportation Enrollment Form/ Bus Discipline Contract/ Consent for Bus Video Tape Release
  • Authorization For Internet Access
  • Transfer of Educational Records
  • Nurse Form – Health History
  • Parent Consent Form for release of Students
